Serious Adverse Events

The Health Quality and Safety Commission (HQSC) produces an annual report into serious adverse events which have generally resulted in harm to patients in District Health Board (DHB) hospitals.

A serious adverse event is one which led to significant additional treatment, is life-threatening or has led to an unexpected death or major loss of function.

DHB providers are required to review these events and report them to the Commission.
