The Health Quality and Safety Commission (HQSC) produces an annual report into serious adverse events which have generally resulted in harm to patients in Te Whatu Ora district hospitals.
A serious adverse event is one which led to significant additional treatment, is life-threatening or has led to an unexpected death or major loss of function.
Te Whatu Ora providers are required to review these events and report them to the Commission.
